Ordering a Script
Patients with ongoing medical problems require regular monitoring – if you have not been seen for some months, you may require an appointment before a script can be written.
Opioid medications: Requests will not be accepted – a face to face appointment is required. No prescription will be provided to any new patient to our practice.
Getting Your Results
Change of Address
If you change your name, address or telephone number, please let our receptionists know by calling or coming in in person. We require emergency contact details as well.

Violent or Abusive Behaviour
We do not tolerate any threatening, abusive or violent behaviour towards any of our staff or patients. If a patient is violent or abusive they will be requested to leave the premises.
